Patient Forms & Info

At Peak, your therapist will listen to you before they make any clinical decisions.  Peak is here to help you achieve your goals and get you back to enjoying life as quickly as possible. Your therapist will take a thorough medical history and depending on the diagnosis or symptoms, will evaluate flexibility, strength, balance, coordination, posture, and body mechanics used in daily activities or work. Effective and efficient manual therapy techniques such as manipulation and Trigger Point Dry Needling may be utilized to speed up your recovery time. Detailed exercises and strengthening programs will be provided based on specific findings during your assessment, and your therapist will educate you, and help you transfer these new skills learned in therapy to improve your quality of life. As therapy progresses, your therapist will re-evaluate how much you have improved and stay in constant contact with your doctor to ensure that your goals are met as quickly as possible.

We want physical therapy to be easily accessible to as many people in our community as possible. In most cases, you do not even need a doctor's prescription to see us. You CAN simply walk in and ask us for help, according to Colorado law. Patients with Medicare do need a prescription from their physician.
